Work Hours/Shift: ⢠Full Time; 8-5 M-F You Will Be Responsible For: · Coordination of network integrity initiatives in coordination with appropriate orthopedic care team members. · Evaluates patientâ™s follow-up needs, as identified through network integrity initiatives, and ensures proper integration into a follow-up program within the AdventHealth CIN · Completes follow-up assessments and evaluations. Investigates questionable situations. · Evaluates effectiveness of network integrity initiatives and facilitates modifications, as needed. · Develops a process for documenting and distributing regular feedback to involved parties about patients who have received Orthopedic Care from within the system of Orthopedic Integrated Health model at AdventHealth · Evaluates, monitors, and assists with recommended levels of care post discharge. · Gathers, analyzes, and presents Orthopedic Integration Program data to the Orthopedic Institute administrative team or other Orthopedic leadership and/or teams to monitor program clinical, operational, quality, and market goals; performs concurrent & retrospective reviews to ensure quality of care. · Presents Orthopedic Program reports and action plans at committee meetings for the AdventHealth System. · Establishes relationships to maximize program effectiveness and facilitates problem resolution across departmental, campus and physician/provider service lines · Ensures that the program is properly communicated in the community covering the Central Florida Division · Remains current in orthopedic and Integrated Health evidence-based practice and new and emerging trends in Orthopedic Care; shares with the Orthopedic Care Team. · Collaborates with the interdisciplinary team to affect change across the system and implement interdepartmental initiatives to improve clinical care processes and ensure best outcomes. · Facilitates in communication among departments and role model critical thinking by applying active and proactive problem solving to complex clinical and operational issues (i.e. deviation from protocol/processes, Orthopedic patient issues, staff and patient educational needs, customer service issues, etc. Qualifications What Will You Need: KNOWLEDGE AND SKILLS REQUIRED: · Advanced knowledge of Orthopedic nursing and health integration theory and practice · Strong communication, collaboration, and conflict resolution skills · Team facilitation and performance improvement techniques · Aptitude for critical thinking and decision making · Ability to manage multiple competing priorities · Knowledge of system and change processes · Leadership skills, creativity, organization, and self-direction · Ability to interact appropriately with patients, families, staff, physicians, and the community. · Skill in using specialized equipment in respective area of practice · Sensitivity to cost containment measures creating an environment that encourages this practice by conserving hospital supplies, equipment, and human resource KNOWLEDGE AND SKILLS PREFERRED: · Data Analysis · Program Development EDUCATION AND EXPERIENCE REQUIRED:

Job Summary: The Advent Health Orthopedic Institute's Network Integrity model is structured to achieve established objectives using a team approach supported by a system-wide Orthopedic Network Integrity Navigator. The Orthopedic Network Integrity Navigator, in collaboration with medical and clinical leaders, is responsible for the coordination of multiple network integrity initiatives in orthopedic care across the AdventHealth central Florida division. The Navigator assists in ensuring a high level of data accuracy and quality within the Orthopedic Network Integrity program and that key metrics through the system database are reported accurately. The Navigator will work with community members and professional organizations to ensure proper integrity initiative adherence and education. The Navigator actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
