The Home Based Program Coordinator (HBPC) is an extended care program at the New Mexico VA Health Care System in the Geriatrics Extended Care Line (GEC) that uses an interdisciplinary team, working with primary care providers to deliver healthcare services to patients in their homes, in concert with family and community support. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: 1. United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. 2. Education and/or Experience. The individual must meet at least one of the following requirements below: (a) Bachelor's degree in occupational therapy and two (2) years of experience as an occupational therapist; NOTE: The baccalaureate degree must be from an approved program prior to the AOTA January 1, 2005 decision that the Accreditation Council for Occupational Therapy Education (ACOTE) would only accredit master or doctoral degree programs in occupational therapy. OR (b) Bachelor's degree in occupational therapy and two (2) full years of graduate education in a related field; NOTE: The baccalaureate degree must be from an approved program prior to the AOTA January 1, 2005 decision that ACOTE would only accredit master or doctoral degree programs in occupational therapy. OR (c) Master's Degree or higher in occupational therapy. Individuals must be a graduate of a degree program in occupational therapy approved by the ACOTE or predecessor organizations. This is inclusive of an internship (supervised fieldwork experience required by the educational institution). ACOTE is the only accreditation agency recognized by the United States Department of Education and the Council for Higher Education Accreditation. Degree programs may be verified by contacting the American Occupational Therapy Association website or at their office address: American Occupational Therapy Association, P.O. Box 31220, Bethesda, MD 20824-1220. Foreign Graduates. Graduates of foreign occupational therapy programs meet the requirements of subparagraph 3b(2) if they have a current, full, active and unrestricted license referred to in subparagraph 3e of this appendix. 3. Certification. Candidates must possess a current NBCOT certification as an OT. 4. State Licensure. Candidates must possess a full, current, and unrestricted state license, to practice occupational therapy in a state, territory or Commonwealth of the United States (i.e., Puerto Rico), or in the District of Columbia. 5. English Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Physical Requirements: The physical demands involve primarily walking, talking, driving, using the telephone etc. Strenuous physical activity is not required although the incumbent is expected to withstand pressure and stress as part of their day to day activities. This position may also required light lifting under 15 pounds, light carrying under 15 pounds, use of fingers, both hands required, walking up to 4 hours, standing up to 2 hours, climbing - legs only up to one hour, the ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, Far vision correctable in one eye to 20/20 and to 20/40 in the other , use of computer screens, and hearing aid is permitted. All persons employed in VHA in this occupational series or in another occupational series that are also performing the duties as described in the qualification standard on the effective date of this qualification standard are considered to have met all qualification requirements for the grade held including positive education and licensure/certification/registration that are part of the basic requirements of this occupation. Employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: The full performance level of this position is GS-14. Grade Determination: Supervisory Occupational Therapist, GS-14 Demonstrated Knowledge, Skills, and Abilities (KSAs). The candidate must demonstrate all of the following KSAs: a. Knowledge of evidence-based practices and clinical practice guidelines in multiple professional areas to use these resources to guide the program staff in providing clinically appropriate treatment interventions. b. Skill in interpersonal relationships and conflict resolution in dealing with a diverse range of employees, team leaders, and administrators. c. Ability to manage [resources] for a large, integrated service including not only occupational therapy, but also other focused rehabilitation, ancillary, or other health care services. d. Ability to communicate effectively orally and in writing with a diverse group of professional staff. e. Ability to translate extensive rehabilitation knowledge into cogent and useful policy in complex facilities and/or across multiple sites. f. Skill in providing clinical and administrative oversight [and supervision of] multidisciplinary rehabilitation programs in complex facilities and/or across multiple sites. g. Ability to collaborate with strategic planning committees at local, VISN or national levels for new ventures addressing patient care delivery systems, facilities management, and system reorganizations. ["Duties will typically include but are not limited to: HBPC provides care primarily to homebound or bed-bound patients. Census includes patients with multiple interacting chronic medical problems requiring prolonged intervention to maintain status and to retard decline, palliative care or terminally ill patients who are not ready for hospice but require home support, and short-term convalescent patients who need healthcare teaching and monitoring. The age range of patients may be from 17 to 100+ years but is predominately elderly. The incumbent provides the full range of supervisory duties which would include responsibility for assignment of work to be performed; performance evaluations; selection of staff, training of staff, and recommendation of awards, advancements, and when appropriate, disciplinary action. The incumbent coordinates development and implementation of the HPBC Program specific to HBPC staff assignment. The incumbent coordinates program implementation with multiple clinical services on campus and Santa Fe, Gallup, and Rio Rancho locations within the New Mexico VA Health Care System (NMVAHCS), including but not limited to nursing, physical therapy, occupational therapy, and social workers; and mental health and primary care providers. The incumbent may provide clinical interventions to the Veteran and caregiver populations served by the program. The HBPC Manager is also responsible for overall oversight of the HBPC Program and will manage and supervise all aspects of the program. This includes but is not limited to management and oversight of the Interdisciplinary Care Planning Process, Joint Commission standards, and ongoing assessment of home care services.
